Abstract

This version of the Small Steps, Big Vision (Grab and Go Guide) has been adapted for use by services supporting children and young adults. The adapted version was co-produced with staff at St Oswald's Hospice. There are two versions in this file: one that shows examples in action from St Oswald's Hospice, and one that is customisable.

The original Grab-and-Go guide builds on research about how people use the Ambitions for Palliative and End of Life Care framework. The research was conducted by a team at The Open University. The research examined case studies and uncovered what helps and hinders people to realise the Ambitions. We found a wide range of practices and confidence levels when it came to using the framework, with many people not knowing where to start or how to progress. A focus on foundations enabled people to work towards multiple Ambitions.
