Abstract

An inner London borough, in partnership with the Foundation for People with Learning Disabilities, undertook a pilot project to promote the introduction of person centred planning (PCP) and develop an associated aggregation tool to help commissioning processes become more responsive to what local people with learning disabilities were saying through PCP. Evaluation of the project considered PCP in its historical and policy context and identified wider lessons from other individualised systems. This chapter aims to share learning by describing our steps towards the development of a workable model and the issues that arose in relation to care management and clinical support systems, leadership styles and commissioning methods.
